Student Forms
To receive interview instructions for any open work study position, bring your completed general application material (section 1) to the Federal Work Study (FWS) Coordinator in Rhodes Hall, Room 143.
Students who do not currently have an FWS award but who are wishing to apply for one should bring this same material (section 1) to the FWS Coordinator to determine their individual eligibility for the program. Keep in mind that approval of such a request is not guaranteed and is decided by the availability of funds and the financial aid eligibility of the individual.
1) Application Cover Sheet
The general application material provides the Work Study coordinator with your basic contact information and most recent employment history. If you wish to list additional work experience, please bring with you a resume. The FWS Agreement must also be completed by each student acknowledging the fundamental guidelines of the program.

2) Payroll Forms
Income earned through the Federal Work Study program is taxable income. Subsequently, any newly hired student must complete the necessary state, local, and federal tax forms.

3) Direct Deposit
Work Study students receive their pay via direct deposit. Pay dates are the 15th and last day of each month. The first paycheck issued after you submit your form will be a paper check you must pick up in the Payroll Office. This provides adequate time for Payroll to verify your account and all checks after your first will be direct deposited.

4) Institutional Forms
Work Study students have the opportunity to be employed in a variety of roles at Columbus State and within the Columbus community. It is the policy and practice of the college that each applicant is screened for criminal background and signs a confidentiality agreement.

5) Work Study Resignation Form
Students wishing to resign from their current work study position much complete this form and return it in person to the Work Study Coordinator in the Financial Aid Office. Students should give a minimum of a two-week notice to their department/agency supervisor prior to their final date of employment. A supervisor signature is required for the form to be accepted.
